Non-small cell lung cancer (NSCLC) is the leading cause of cancer-related deaths worldwide. In the Phase III HARMONi-6 study, ivonescimab has achieved significant and clinically meaningful improvement in overall survival as a first-line treatment for advanced squamous non-small cell lung cancer patients. The study results were published in The Lancet and were presented in a plenary session at the 2026 ASCO Annual Meeting.
Patient Enrollment and Trial Design
In HARMONi-6 study, 532 patients were enrolled and among them, about 63% had centrally located squamous tumors. This randomised trial was conducted at 50 hospitals across China. Patients aging 18 to 75 years with previously untreated, pathogically confirmed, unrepeatable stage IIIB, IIIC or stage IV squamous NSCLC and an Eastern Cooperative Oncology Group performance stage score of 0 or 1 were eligible for inclusion.
Treatment Groups and Safety Findings
The patients were randomly assigned in a 1:1 ratio to receive ivonescimab or tislelizumab, in combination with paclitaxel and carboplatin for four cycles. According to Lu, baseline characteristics were balanced between the ivonescimab and tislelizumab arms.
Ivonescimab plus chemotherapy showed a safety profile consistent with prior reports. Treatment-related adverse effects occurred in 99.2% of patients in both arms and 69.2% of patients in the ivonescimab arm experienced grade 3 or higher TRAEs vs 58.9% of patients in tislelizumab arm.
Global HARMONi-6 Study Expands Research
The global phase 3 HARMONi-6 study is examining ivonescimab-based therapy in a broader, international NSCLC population. The trial will examine ivonescimab combined with chemotherapy vs pembrolizumab combined with chemotherapy in those with first-line stage squamous or non-squamous NSCLC.
Potential New First-Line Treatment Option
This regimen could provide a novel treatment option as first-line treatment in this patient group. This study was not powered to demonstrate superiority for overall survival.
